Membership Terms & Conditions
- *All Appointment bookings are subject to GP availability.
- Only exclusions are HFH visiting Consultants or extra GP fees, retail products/others not listed as part of benefits.
- Initiating member must be aged 18 years or over.
- Cancelling your membership must be done in writing and there is a 2 month notice period, please write to: Membership, Harley Fitzrovia Health, 253 – 269 High Road, Woodford Green, Essex, IG8 9FB.
- Your membership will automatically renew annually unless notice to cancel is given.
- Harley Fitzrovia Health reserves the right to cancel, suspend or withdraw membership at any time without notice.
- Harley Fitzrovia Health reserves the right to change these terms and conditions without notice, but you will be notified in writing.
